| The association is providing this form for your use in an effort to make it easier to register and more easily resolve community and neighbor related problems and inform the individual(s) of them. In the spaces below please fill out in detail what you are requesting we report and record. These details will make it easier for the homeowner(s) etc. to comply or respond to the problem. Remember, if the problem involves immediate danger, it is requested that you call the police or fire department instead of using this form. A copy of the form will (excluding the lower area) be mailed or given to the resident or group you name. The lower section with your name | will not be mailed or given out, only filed for records. After 3 forms have been filed about a situation in a 12 month period, the board will review the situation and, if necessary, take action as specified in the Coronada handbook This method allows the individuals or groups to correct the problem or to respond to us if they feel the report is in error or the request unreasonable. After filling out, drop the form in the Coronada requests mailbox. Anonymous forms may be filed, but they will only be recorded, not mailed or given to the individual(s) named. The association can not mail problem requests or assist in resolution unless this form is completed. | |
